When it comes to decision making in your forum, do you make any attempts to discuss new policies with your administrative/mod team before making the final decision or do you make your decision and tell them to carry things up from it immediately?

I feel that if it's a new policy that's going to affect the whole community, it's best to discuss it with your team to hear about their views on it before making a decision because it's more likely going to be how it's received by the general members.

In my opinion consulting with others is a must in every aspect of life especially when it is related to many people, by consulting with others you will have a combination of several great ideas.

Depends on what kind of features you are trying to implement changes. AS a moderator, I discuss with admins and other staff. However, as a owner and admin I make all decisions

I feel as an owner and admin of a forum, you need to always be in contact with your staff and any decisions or ideas that people may have, do need to be brought to the attention of all staff for discussion before anything is implemented.

A lot of the time, there are no issues with something being implemented but I do feel that bringing it to the attention of your staff for input and just so they know there is going to be a change is the decent thing to do.

I feel the same way too because it's something that's going to make all your staff members feel like they are being carried along and not like you're running the forum as an army general who just decides and implement whatever he needs. If one decides to manage his forum in that manner, there's no need for having staff members.
