I don't know if there is a format for presenting a letter of complaint about a business. But I know the content should be exactly what wrong was done to you. You should not lie about it lest you put employees or the business in huge problems.

Yes including the exact date it happened would go a long way to make things easy for you. The company would be able to trace that sales on time if they are provided with the date, and in most cases some do ask of the time self if possible.
