How quick are you to issue a ban to someone for their behavior or for violating the rules?

How quick are you to issue a ban to someone for their behavior or for violating the rules on your forum? What steps do you take before issuing a ban?
I give a few warnings. Typically it begins with a verbal warning, then becomes an official one if the same behavior occurs again. Depending on how long it is til they do it again, I may warn them one more time and then after that a ban. If the person is doing it over and over, after the warning, even on the same day of the warning, I would likely issue a ban.

I really do not enjoy banning users - but sometimes it is the only option.
 
If its a minor rule infraction, 1 strike. If its a major rule breakage, its 2 strikes. At 3, they are banned. Points expire in roughly a month, although bans are permanent. We must be more strict since we run a PG-rated community.
 
Unless it's something severe like mass spamming, flaming, or illegal matters I'm not too quick to ban people. I usually go by the "three strikes and you're out" rule of thumb. If someone comes to post one spam advertisement and leaves I usually do ban them because it's obvious they registered just to advertise only. In fact, I only have two banned members on my board of almost 1500 members and that is the reason they were banned; one spam advertisement.
 
You only can be as fast if you are online when it happens. Many years ago I had a troll who was trolling me so badly by making many accounts and changing his IP address and emails. I played his game till he got so damn bored that he/she stopped. But it impacted the site badly one night. I was up in the morning and visited the site and all I see was 100s of pornography images posted everywhere. I ban the user right away and I had to close the site down for an hour to clean the site up.
