before I ban any member I give him a warning, I'll give me three warnings before I ban him, with each warning i'll limit his activities in my forums, for exemple in the first warning he can't send pms or reply or create threads for 3 days, the next warning will be longer, and the third is longest then I'll ban him for sure
 
I have only had to ban a member twice or thrice in my forumming experiences, on forums I have ran. The times I have had to ban users, were because they were trolling/spamming and they were literally building a bad reputation for my forum.
 
Warnings are ok. But again, it all depends on what he/she did. If he did something which is strictly, clearly and obviously forbidden in the rules, then, I believe, there is no point in giving him more than one warning.
 
You need to write down your form rules clearly and send the link to the member after registration so that there is no confusion in the future. When your member violate rule, you will have to send hims a message telling him what violation he has committed. Second violation can be a ban
 
I'm not a staff member on any site. If i were to be one, i woud ban members if i would have given them three warnings and they still violated the rules of the forum after the warnings.
 
I ban members only for the grave mistakes, something like violating rules, spamming forums, harassing other forum members, plagiarising content, posting suspicious links, etc.
 
I never appreciate hate speech on my forum, criticisms are encouraged however if someone inulges in hate speech then I'll ban them for an indefinite period without any warning.
