Have you ever lost access to your wallets?

I have never lost access to my wallets, since I have always known that it is vitally important to properly save my seed phrases and have passwords that I know I will remember so as not to lose access to my cryptocurrencies. In your case, have you ever lost the access to your wallets? What did you do when you realized it?

Lost This happened with my very first Metamask wallet, I didn't have a lot of funds then, but when I needed to withdraw them, it turned out that I lost a note where all the phrases from this wallet were written down.
